- Comfortable working with diverse patient populations, including the elderly.
Responsibility
- Learn the Solace systems, tools, technology, partners, and expectations while providing clinical expertise.
- Identify and prioritize patients’ needs to maintain a streamlined care continuum.
- Oversee patient care plans in partnership with healthcare advocates.
- Address social determinants of health such as food resources and transportation access.
- Facilitate an unparalleled advocacy experience for patients.
