They are seeking a compassionate LPN/LVN to join their remote team, providing Remote Therapeutic Monitoring (RTM) and Chronic Care Management (CCM) services. The role involves patient engagement, health documentation, and support, all while adhering to HIPAA regulations.
Qualification
- Certification as an LPN
- 3+ years of experience working in office or remote settings
- Proficiency with technology such as Zoom, Slack, and online enrollment platforms
- Strong organizational skills and ability to manage multiple tasks efficiently
- Excellent communication skills, both verbal and written, with a focus on empathy and patient support
- Ability to work independently in a remote setting, showing initiative and self-motivation
- Familiarity with HIPAA regulations and commitment to maintaining patient privacy
Responsibility
- Facilitate patient enrollment into RTM and CCM programs using Flagler’s portal.
- Make daily calls and text messages to patients to document health status and keep them engaged.
- Assist in the collection and documentation of patient health information, including histories and new symptoms.
- Support patients by sending health pointers such as stretches and meditation videos.
- Maintain strict confidentiality in handling patient information, adhering to HIPAA regulations.
