FreedomCare Pennsylvania is building a talent pool of customer-service professionals for multiple home-care oriented roles (HomeCare Sales Specialist I, Intake Specialist I, Onboarding Specialist I, Care Support Specialist I, and Field Care Coordinator). Roles are primarily phone-based with in-person travel for Field Care Coordinators, focused on compassionate support, enrollment, onboarding, and ongoing care coordination for patients and caregivers in a healthcare setting.

Qualification

  • Proven track record of delivering high-quality customer service with empathy and patience.
  • Ability to manage high-volume inbound and outbound call queues.
  • Exceptional verbal and written communication skills with the ability to convey empathy.
  • Strong emotional intelligence and teamwork, with the ability to collaborate in a fast-paced environment.
  • Problem-solving skills with the ability to resolve issues over the phone and de-escalate frustrated callers.
  • Excellent time management and multitasking capabilities.
  • Willingness to travel as needed for Field Care Coordinator duties; flexibility to work across multiple roles in a healthcare setting.

Responsibility

  • Engage with individuals over the phone to build relationships and assess home care needs.
  • Educate and assist patients and caregivers through the application, enrollment, and onboarding processes, ensuring eligibility is confirmed.
  • Guide patients and caregivers through onboarding and orientation for FreedomCare programs.
  • Provide ongoing care support via phone, helping patients receive necessary services and feel heard and valued.
  • De-escalate difficult calls and resolve issues with empathy, professionalism, and timely follow-up.
  • Manage high-volume inbound and outbound call queues with strong time management and organizational skills.
  • Collaborate with internal teams to coordinate care and communicate updates; Field Care Coordinators travel to patients' homes as needed.
