NeuroNav is seeking remote Care Managers to connect with families of adult neurodivergent children, helping them access resources and navigate a California state-funded program called Self-Determination. The role involves creating personalized care plans and advocating for clients' needs, contributing to the mission of enhancing the quality of life for adults with developmental disabilities.
Qualification
- Experience in social work, case management, or service coordination
- Strong relational skills and ability to build relationships with families
- Creativity in developing personalized care plans
- Ability to manage budgets and advocate for client needs
- Experience in project management and coordinating with multiple stakeholders
Responsibility
- Person-centered Planning to capture clients' unique strengths and goals
- Budget & Spending Management to advocate for personalized support needs
- Project Management to guide clients and partner organizations through processes
- Service Provider Access to assist in implementing person-centered plans
- Contributing insights to Special Projects based on client interactions
