Qualification

  • Experience in social work, case management, or service coordination
  • Strong relational skills and ability to build relationships with families
  • Creativity in developing personalized care plans
  • Ability to manage budgets and advocate for client needs
  • Experience in project management and coordinating with multiple stakeholders

Responsibility

  • Person-centered Planning to capture clients' unique strengths and goals
  • Budget & Spending Management to advocate for personalized support needs
  • Project Management to guide clients and partner organizations through processes
  • Service Provider Access to assist in implementing person-centered plans
  • Contributing insights to Special Projects based on client interactions
