Qualification
- Board certification in family medicine, internal medicine, or other relevant specialty.
- Strong clinical skills with excellent organizational abilities.
- Superior communication skills, emphasizing listening and empathy.
- Comfortable working with elderly patients and individuals facing social determinants of health.
- Technical savvy with the ability to quickly learn new systems and tools.
Responsibility
- Learn the Solace systems, tools, technology, partners, and expectations while providing clinical expertise.
- Identify and prioritize patients’ needs to maintain a streamlined care continuum.
- Oversee patient care plans in partnership with healthcare advocates to address social determinants of health.
- Facilitate an unparalleled advocacy experience for patients.
- Collaborate with healthcare advocates to help patients achieve their health goals.
