FreedomCare Pennsylvania is building a talent pool of customer-service professionals for roles in home-based care, including HomeCare Sales Specialist I, Intake Specialist I, Onboarding Specialist I, Care Support Specialist I, and Field Care Coordinator. These roles focus on helping patients and caregivers via phone, guiding enrollment and onboarding, and, for Field Care Coordinator, conducting in-person visits to coordinate care.
Qualification
- Experience in customer-service roles with high-volume call handling (inbound and outbound).
- Excellent verbal and written communication skills with the ability to convey empathy and compassion.
- Strong emotional intelligence, active listening, and teamwork skills.
- Proven problem-solving abilities and ability to de-escalate difficult callers.
- Effective time management and multitasking abilities in a fast-paced environment.
- Ability to educate and guide patients/caregivers through enrollment and onboarding processes.
- Willingness and ability to travel for Field Care Coordinator responsibilities (in-person visits).
Responsibility
- Handle high-volume inbound and outbound calls to engage individuals seeking home care or enrollment.
- Educate and assist patients and caregivers through the application and enrollment process for supported programs.
- Guide patients and caregivers through onboarding and orientation to FreedomCare.
- Provide ongoing care support with empathetic, clear communication to ensure patients receive the care they need and feel heard.
- Collaborate with patients, caregivers, and care teams to coordinate care, confirm eligibility, and facilitate program enrollment.
- Support Field Care Coordinator duties with in-person visits to patients' homes as needed.
- Document interactions and maintain accurate records in internal systems to support continuity of care and compliance.
