Qualification
- Registered Nurse (RN) license in New York State
- Experience in home care or community health preferred
- Strong communication and interpersonal skills

Responsibility
- Provide education, support, and resources to caregivers to help them become certified Health Home Aides (RN Nurse Educator)
- Conduct patient assessments to determine care needs and develop personalized care plans (RN Field Nurse Assessor)
- Support the implementation of care plans and ensure quality of care delivery
- Collaborate with other healthcare professionals to enhance patient care
- Participate in training and development programs for staff
