Qualification
- Registered Nurse (RN) license in New York State
- Experience in nursing education or patient assessment

Responsibility
- Provide education, support, and resources to caregivers to help them become certified Health Home Aides (RN Nurse Educator)
- Conduct patient assessments to determine care needs and develop personalized care plans (RN Field Nurse Assessor)
- Support the implementation of care plans in collaboration with caregivers and patients
- Participate in training and development programs for clinical staff
- Ensure compliance with healthcare regulations and standards
