Qualification
- RN license in good standing.
- Deep understanding of Social Determinants of Health and experience with diverse patient populations.

- Technical savvy and ability to quickly adapt to new technologies.
Responsibility
- Learn the Solace systems, tools, technology, partners, and expectations while providing expertise in interactions.
- Build strong, trusting relationships with Medicare patients based on listening and empathy.
- Identify and prioritize Medicare patients’ needs to maintain a streamlined care continuum.
- Develop comprehensive patient care plans addressing social determinants of health.
- Collaborate to build future systems for working with Medicare patients.
