BridgeHealthAI is a fast-growing company focused on improving patient care coordination for underserved populations. They are seeking a part-time Patient Care Operations Coordinator to assist with patient support and benefits navigation in Massachusetts. The role involves outreach, enrollment in benefits programs, and communication with patients, particularly those from lower-income backgrounds.
Qualification
- Experience in patient enrollment or benefits navigation as a CHW, Navigator, CAC, Patient Access Specialist, or equivalent role
- Ability to communicate effectively in English and Spanish
- Empathy and strong communication skills to assist lower-income patients
- Familiarity with healthcare benefits systems, particularly Medicaid and MassHealth
- Ability to work part-time on a contract basis for 3 months with potential for extension
Responsibility
- Conduct high-volume outreach to patients and community organizations via phone, SMS, and email
- Assist patients in Spanish when needed
- Enroll and assist patients with MassHealth, Medicaid, and ConnectorCare applications and renewals
- Identify eligibility for programs such as SNAP, WIC, LIHEAP, RAFT, EITC, etc.
- Use internal tools to manage structured patient task queues efficiently
- Verify insurance coverage and help patients understand next steps
- Document outcomes and track patient progress
